Several classic weapons return, including the Painkiller, the stakethrower, the shotgun, and the electrodriver. Each level presents a new location with various themes and includes castles, monasteries, graveyards, and more. The game is divided into several chapters, each with several levels. The player has to get through each level, from start to finish, by slaughtering multitudinous demons. Painkiller: Hell & Damnation received a mixed reception among gaming publications. It was released in North America for Xbox 360 through Xbox Live on October 1, 2013, and for PlayStation 3 through the PlayStation Store on November 26, 2013. The game was released on October 31, 2012, for Microsoft Windows and for PlayStation 3 and Xbox 360 on June 28, 2013, in Europe after suffering multiple delays. The original PvP multiplayer modes are still present and the game is described as uncut with none of the gore or blood removed.Painkiller: Hell & Damnation is a first-person shooter video game, both a remake of and a sequel to Painkiller, developed by The Farm 51 and published by Nordic Games. It also introduces a survival mode where up to eight players can fight together to survive waves of enemies. Entirely new is a cooperative mode, both local and online, where the protagonist Daniel Garner can team up with Eve to tackle the levels together.

The original music is complemented by some new tracks by Ojo Rojo. It combines a saw blade with an energy beam to suck up the souls of defeated enemies right away. Next to the original weapons the Soulcatcher is introduced. The game starts with a new intro cinematic that changes the setup of Daniel Garner's trip to purgatory. It is the second time a game in the series is available for consoles, following the 2006 Xbox title Painkiller: Hell Wars. Most of the visual enhancements are apparent in the character models and skins.

There is also a global graphical overhaul courtesy of converting the game from the original PAIN engine to UnrealEngine3. Each chapter has a boss and they were reconstructed with changes to the level design and the enemies. Of the 34 levels originally available in those games, 14 are featured in this title reorganized in four chapters. Painkiller: Hell & Damnation is an enhanced remake based on the original Painkiller and the expansion Painkiller: Battle Out of Hell.
